EVEREST PALLIATIVE CARE

Everest Palliative Care is for those diagnosed with a serious disease who may be getting vigorous treatment and want home-based education and care. Our social worker will come to you and connect you with community resources. There is no charge for this service.

Social workers provide information about the disease, community resource referrals and support to navigate the healthcare system.

On-duty nurses are available 24/7 to answer questions and offer support.

The results: Comfort and peace of mind.

WHAT IS PALLIATIVE CARE?

Palliative care focuses on supporting the patient in a holistic way- meeting physical, psychological and spiritual needs.

The goal is to achieve the best quality of life possible.

People can receive palliative care at the onset of diagnosis, and even while receiving treatment for their chronic condition.

Palliative care provides education, assistance and reassurance. We work in collaboration with your physicians to determine and create the best plan of care based on your needs.

WHAT WE PROVIDE:

Working in collaboration with your physician, provide s the following services in your home or place of residence:

  • Education about your disease process

  • Family caregiver education

  • 24/7 telephone support from a nurse for medical questions or concerns; in the event of an emergency, please call 911

  • Referrals to community resources

  • Help navigating the healthcare system

  • Assistance with living wills and medical Power of Attorney forms

  • No-cost placement service referrals, if needed

OUR PATIENTS

People who would benefit from Everest Palliative Care are those who have a serious illness that has prompted multiple hospitalizations in the last year and have one or more advanced health conditions.

An appropriate palliative patient may be seeing multiple providers and seeking multiple therapies that may cause adverse side effects.

CHRONIC MEDICAL CONDITIONS INCLUDE, BUT ARE NOT LIMITED TO:

  • Cancer

  • Heart disease

  • Lung disease

  • Liver failure

  • Kidney failure

  • Neuromuscular disease
